Confidential Intake Form
1 page document: standard form outlining medical history and agreement to description of services. Downloading, printing and filling out this form will remove the need to fill one out before the session starts.

Policies and Insurance Information Form
1 page document: outlines our cancellation policy and determines eligibility of payment through insurance. Downloading, printing and filling out this form will remove the need to fill one out before the session starts.
